Award Nomination Form

All nominations must be submitted using the MoHIMA Awards Nomination Form, completed in its entirety. Nominations must follow the nomination guidelines and must include the supporting materials listed below.

The nominee’s resume and two reference letters must be uploaded on the online nomination form.

Awards will be presented at the MoHIMA Annual Meeting.
